1. Has the company had a history of worker fatalities, serious injuries, or been cited for a serious or willful violation of OSHA, MSHA, or other violations under applicable international, federal, state, or local health and safety or labor laws within the last 3 years?
2. Does the company provide initial and continuous occupational health and safety training to workers in their primary language?
3. Does the company regularly audit its existing manufacturing facilities and operations for potential safety issues?
4. Does the company include records of work-related injuries, illness, or disease in its annual corporate sustainability report or other method of public reporting?
5. Does the company have occupational safety and health committees with full worker participation, where workers are involved in preventive activities and evaluation and implementation of alternative products, materials, and practices?
6. Has the company proactively identified and implemented engineering and/or design measures that prevent worker exposure to both physical and chemical hazards in the workplace?
7. Does the company have an ongoing program or committee that meets regularly to develop, evaluate, monitor, and review strategies and initiatives to proactively address occupational health and safety issues in the workplace?
8. Has the company implemented a redesign of the product or modification of its production process that has improved worker safety through elimination of potentially hazardous chemicals, processes, or equipment from the workplace?
9. Does the company require similar standards/programs and public accessibility from its supply chain, such as is required under Occupational Health and Safety Assessment Series (OHSAS) 18001 standard or Social Accountability International's SA 8000 standard and verification systems?
